Honestly I’d say mechanics and people failing depends on the day and the time. I tend to see better groups on reset and during the AM. They get worse by Sunday.
While doing ARR leveling dungeons for my DPS I've found Dzemael Darkhold to go very badly sometimes, especially if the tank is new. The pull after the first boss with the two frogs usually always has a wipe
recently almost wiped in thousand holes of toto rak cuz the tank didnt use defensives and at the ginal boss the 2ps just pulled it while the tank was in a cutscene the entire time (my first time playing healer too)

CC is by far the roulette I've seen the most wipes on.
Endwalker first trial when it spins the entire arena is always giving me the hardest time. I know what im supposed to be doing, but i couldnt visualize the safe spot. Anybody having trouble with this too?

For your testing specifically any hard mechanic is made exponentially worse when it's specifically the tanks + healers (or just 1) is not extremely on top or things or is clueless. I.e. today I had an Emerald weapon that nearly wiped in phase 1.
The Ninja in the Dun Scaith run constantly using Doton… physical pain. (As a genuine question why do people constantly think Doton is good on single target? The potency is so miniscule compared to Raiton.)
It’d be fun to try out multiple data centers – I have a horror story for almost every instance from my crystal alt, the casual player skill there tends to be lower
I've been a tank in raids like this and it's INFURIATING how few tanks turn on tank stance. Yes, I have mine on. You have yours on two in case I die.
Some duties that tend to have rage quitting and/or wipes:
Eyes of the Creator Normal
Level 89 Trial Normal
Patch 5.3 Trial Normal
Orbonne Monastery
End of Days Dungeon
The Tower at Paradigm's Breach
The mudpie add at the last boss in Saint Mocianne's Arboretum (Hard) tends to claim a few victims every run, if not straight up wipes. People will usually all focus on it, hit it near a wall, and at that point it's unrecoverable, at least in my experience. If it gets hit by the boss once, it's gonna hurt. If it gets hit twice, it's probably a wipe without a SCH or SGE.
Similar thing with the last boss of Haukke Manot (Hard) If people don't chill with dps at 40%, and focus on the last add, it's usually a wipe without a shield healer.
2nd and 3rd boss of the Mhach raid can cause wipes, the former because of Mega Death, the latter because of phase transitions, meteors being dropped too close, and those orbs in cube form.
For trials, Kugane Ohashi can cause issues, funnily enough. The aoes into donuts can kill people, the part after the add phase, where all but one spot near the boss is safe can kill those far away, and the chains can claim a few victims, if they are too close to the boss, when they get chained.
The lvl 83 trial can be a troubling one too. He tends to use his multi-stack way more often, than the ex version, and it hurts, especially if 1 or 2 people died right before the stack to another mech.
EDIT: just remembered the Hells' Kier, the bird adds at the start can cause a wipe, if they're not managed properly. DPS usually spread out with them, then at the start, most of them get rezzed by the feathers, and then they go boom, and wipe.
For normal raids, there's a few sketchy ones:
– A5 can wipe when 3 bombs spawn, if boss isn't tanked in the right place, and the bombs aren't knocked away to the right corner,
– A9 wipes, if ot doesn't know where to drag the faust add, and lets it fall into lava too early,
– A11 can wipe, if it gets to Propeller Wind, also the mt charge after its shield gets broken can claim a few victims,
– O11, because lardboard/starboard, but the intermission phase can be troubling too, since some people tend to ignore the the 2nd set of meteors,not cleanse their debuff, and then are made to run into the deathzone, when it expires,
– O12, because of female's knobacks in phase 1, and the general mech overlap mayhem in phase 2,
– E7, because people just can't think with portals,
– I once had to rezz the whole party with heal lb3 in E12, so I'll say that one too can spiral out of control in phase 2
Above all though, I think healers being good/bad is a huge factor to how disastrous some of those runs can be, so You playing a healer is bound to make things more manageable.
With the current moogle tomestone event, any Nier raid became battle royal
A thing people also quite often forget about are the different Burnt Strikes in E11! So often do people either just eat a vuln stack or straight up go flying off the arena… even after they've seen the mechanics for the 3rd time!
'worst' duties in terms of chance of failure are for sure zodiark and hydaelyn. if you instance in and both healers are watching cutscenes it is near guarantee a wipe. zodiark has an extremely nasty (for normal difficulty) heavy-hitting stack after he does instant kill mechanics like the knockback and astral eclipse. the stack will pretty much wipe if 4 people die to the previous mechanic, sometimes even 3

I agree with a lot of these. As an Astro main I’ve encountered the most gong shows via Leveling and Mentor roulette, because you usually end up paired with people unfamiliar with the duty and/or their job abilities. Dun Scaith is the worst offender without question, but also:
– Dzemael Darkhold – the crystal light mechanic
– Aurum Vale – any virgin run
– Lost City of Amdapor – Diabolos’s doors
– Tam-Tara Hard – First boss
– Sunken Temple of Qarn Hard – First boss sandpit, last boss mummification (especially if the tank gets it)
– Bardam’s Mettle – Second “trial” boss
– Anamnesis Anyder – especially the second boss (Kyklops)
– Paglth’an – Lunar Bahamut
– Vanaspati – Last boss
– Ktisis Hyperboreia – Last boss
– Dead Ends – Last boss
– The Whorleater – DPS offing themselves by hitting the wrong part of Leviathan and not noticing
– The Chrysalis – Not grabbing orbs, tanks not taking meteors, not saving LB3 for the Aetherial Tear
– Containment Bay P1T6 – Falling off the scales
– Rathalos Ex – Bane of my Mentor roulette existence
– The Dancing Plague – Floor vines
– Royal City of Rabanastre – Mateus drowning, Hashimal towers, Rofocale dark circles
– Orbonne Monastery – Ultima
– Tower at Paradigm’s Breach – Red Girl & False Idol (Badly placed Lighter Note AOEs, especially)
– Alexander Fist of the Son – Ratfinx
– Alexander Eyes of the Creator – Refurbisher
– Eden’s Gate Sepulture – EVERYTHING
– Eden’s Verse Iconoclasm – EVERYTHING
And I’m mostly just a casual player who follows the herd and/or safety dorito when I don’t remember the mechs.
This was fun. But maybe visit another Data Center. The quality of play is wildly more inconsistent on Primal (there's no mid-runs there, only perfection and disaster) and Crystal's rep goes without saying.
Gonna be hard to see some of these since they're connected to first time/past trauma prior to power creep invalidating a lot of mechanics/bypassing them through killing bosses faster.
But I'll toss out three mechs that I've seen failed fairly consistently even if they don't lead to wipes.
1. Ozma Meteors
2. Healer check final boss of Wanderer's Palace Hard Mode (unless the party mentions it)
3. Voidgates during the Everliving Bibliotaph in Great Gubal Library.
I'm quite old now (66) and mostly, in Duty FInder I'm like Gandalf "I have no memory of this place". It DOES make the game more fun though since I'm very much a casual these days.

Getting Ramuh EX in mentor roulette is the worst
That Dun Scaith run pretty much encapsulates everything about why I main tank.
Also, the reworked final boss for Aetherochemical Research Facility is pretty spicy.
Dun Scaith has always been a problem for me, though usually we do at least have tanks lmfao
The Burn was a problem for me for a long time, until someone finally explained that mechanic to me. The funniest thing is by that point I had already seen the mechanic in E11 so I felt real dumb.
I'm honestly surprised that The Chrysalis hasn't found its way into the comments yet. At least in my experience things can go south there REALLY FAST if no dps bothers to LB3 the add during meteor phase. Just in case you want to do another instance of this video at some point 😉

For me personally the entire list of level 70 alliance raids has a high chance of… not going too well, so I was surprised to find none of them in this video. Maybe my personal experiences were nothing but bad luck.
I'm talking about stuff like the sniping mechanic in Orbonne, the transition phase from the final boss in Orbonne(the final set of bosses spawning), adds not being killed in boss 2 of Rabanastre and such. There is so much stuff that can go wrong, and most of the time alliance roulette throws me there, some of them do xD.
Haukke Manor (Hard) causes a lot of wipes on the final boss as it's one of the few instances where you will lose if you DPS too well.
I'd say a lot of the ARR ones definitely have annoying mechanics. As well as because a lot of us do a lot of damage we also pushes out mechanics much quicker. Like, Haukke Manor (Hard) last boss, you actually have to hold back on dps otherwise you'll push her mechanics out too quick and she'll wipe the party. It especially painful if no one knows, though explaining it to them will hopefully help :p

A lot of these are minor annoyances compared to the actual worst duty which is Seat of Sacrifice.
There’s nothing stopping one person from making that duty impossible to complete, purposely even.
