I always just saw it as the Trial bosses looking at a drawing board and going, “ok, my fight is nice and all, how can I make it as annoying as calculus.” But in all seriousness, how SE does the lore for those is pretty incredible.
Most of these additional difficulties are grabbed from The Wandering Minstral. Those difficulties are meant to be the Minstral telling the story of your fight, and like any bard, the story may be a bit….exaggerated lol
I love the ones like the Warring Triad extremes, where it's referred to as the captors making a test chamber for you, where they are remade to be as strong as they were before, instead of just waking up and now you have to stop them. It's also fun when you unlock the last one and the one unlocking it for you just says "yeah You're gonna die buddy, thanks for your sacrifice"
