This video is based on the recent 0.0.0.6 version, so likely future versions may remove the need to configure the steam VR resolution in the config file before playing as well as other quality of life additions.

Download FFXIV from either Steam or directly from Square Enix.
Download the XIV launcher plugin from the below link:
https://github.com/goatcorp/FFXIVQuickLauncher
While installing refer to where you installed the main game. Within the plugin launcher ensure Dalamund plugins are enabled.
Open SteamVR and check your per eye resolution. Open the FFXIV.cfg with notepad normally found in Documents/My Games/Final Fantasy XIV – A Realm Reborn and type in the screen width and screen height that you set in Steam VR. Start low and raise over time till you get good results.
Some recommend graphics settings can be found on the discord, but definitely start low and raise the graphics till you find a good point. Ambient Occlusion is especially heavy and can save you around 40fps. You are aiming for 180fps for a smooth 90fps.
When you launch the game youโll see the Dalamund red plugin icon in the corner on your PC monitor. Open it and enable experimental plugins. You no longer need to link to the direct repo, just turn on experimental plugins and search for XIVR. Over time it will likely be added to the non-experimental branch.
Plugins in general are a grey area, don't talk about them in game and you'll generally be okay. If you have 10k hours and spent a lot on cosmetics maybe think twice about the risk, however new players needn't worry as there is a very generous free trial.
Thanks for the video!
I got a question, is it possible to play with mouse and keyboard as well?
I believe everyone prefers playing on a controller when in a VR but is it possible to keep my HUD setup for mk?
Awesome. I was trying the game last night and had to quit after like 10 minutes from eye strain and headaches. I would have never thought to do all that cofig file stuff! Thanks for the vid!

4090 and still only 30 fps
Do you turn off vsync in nvcp? My in-game fps caps at my oculus frame rate unless i turn it off
Thank you for the guide. I followed the guide and was able to run the game in VR. Now, in game FPS counter shows me around 130-150 in a town and I still experience massive stuttering. So you're saying I need to get 180 FPS for smooth "90hz" game experience? It is odd that my cpu and gpu are utilized only about 20%. How do you get constant 180 FPS?
